| 摘要： | 目的：为甲基苯丙胺依赖患者脱毒治疗期间继发医院感染的预防及治疗提供参考。方法：回顾性分析我院2011年11月－2015年10月318例甲基苯丙胺依赖患者脱毒治疗期间的临床资料，分析感染率、感染部位、感染危险因素、病原菌分布特征等相关因素。结果：318例甲基苯丙胺依赖患者脱毒治疗期间继发医院感染122例，感染率为38.4％。患者医院感染部位主要分布在呼吸系统和消化系统；病原菌主要以肺炎克雷伯菌、肺炎链球菌、金黄色葡萄球菌为主；合并基础疾病、血清白蛋白水平低、有侵入性操作、抗菌药物使用、白细胞计数低的患者发生医院感染率较高，分别为53.5％、58.9％、47.8％、47.4％、45.9％，是医院感染的危险因素，差异有统计学意义（P＜0.05）。结论：甲基苯丙胺依赖患者脱毒治疗期间继发医院感染率高，医务人员应积极处置基础疾病，严格且尽可能减少各项侵习性操作，采取多种措施综合干预来减少医院感染。 |
| ABSTRACT： | OBJECTIVE： To provide reference for the prevention and treatment of secondary hospital infection in patients with methamphetamine dependence during the detoxification treatment period. METHODS： The clinical data of 318 patients with methamphetamine dependence in our hospital during detoxification treatment period were analyzed retrospectively from Nov. 2011 to Oct. 2015 in terms of infection rate， the site of infection， infection risk facter and distribution characteristics of pathogenic bacteria and other related factors. RESULTS： Of 318 methamphetamine dependent patients during detoxification treatment period，122 suffered from secondary hospital infection with infection rate of 38.4％. Infection sites mainly involved respiratory system， digestive system； main pathogens were Kelebsiella pneumoniae， Streptococcus pneumeniae and Staphylococcas aureus； high hospital infection rate occurred in patients with combined underlying disease， low albumin level， invasive operation， antibiotics use， low white-cell count， accounting for 53.5％， 58.9％， 47.8％， 47.4％ and 45.9％， respectively. They were risk factors of hospital infection， and there was statistical significance （P＜0.05）. CONCLUSIONS： High secondary hospital infection rate is found in methamphetamine dependent patients during detoxification treatment period， medical staff should actively handle the basic diseases， strictly minimize all kinds of invasive procedures and take multiple measures intervention to reduce hospital infection. |
