| 摘要： | 目的：比较4种无痛人工流产术麻醉方案的麻醉、镇痛效果及安全性。方法：200例自愿接受无痛人工流产术的患者按照随机数字表法分为A、B、C、D共4组，每组50例，分别采用生理盐水+丙泊酚（A组）、枸橼酸芬太尼+丙泊酚（B组）、地佐辛+丙泊酚（C组）、氟比洛芬酯+丙泊酚（D组）4种方案进行麻醉。比较4组患者麻醉后的实验室指标、麻醉和镇痛效果以及不良反应发生情况。结果：D组患者术中舒张压和术后心率均高于A组，术中血氧饱和度高于A、B、C组，差异均有统计学意义（P＜0.05）；D组和B组患者丙泊酚用量显著少于A组和C组，差异有统计学意义（P＜0.05）；D组患者苏醒时间较短，与A组比较差异有统计学意义（P＜0.05）；D组患者视觉模拟评分（VAS评分）明显低于A组和C组，差异有统计学意义（P＜0.01或P＜0.05）；D组患者满意度较高，与A组比较差异有统计学意义（P＜0.05）；D组患者不良反应发生例数少于其他各组，差异有统计学意义（P＜0.05）。结论：氟比洛芬酯用于无痛人工流产术的镇痛作用显著、丙泊酚用量少、患者苏醒时间短、不良反应少，是一种安全有效的麻醉方案。 |
| ABSTRACT： | OBJECTIVE： To compare anesthesia and analgesia effect， safety of analgesia regimen among 4 kinds of painless induced abortion. METHODS： 200 women undergoing painless induced abortion were randomly divided into group A， B， C and D， with 50 cases in each group. They were given normal saline+propofol （group A）， fentanyl citrate+propofol （group B）， dezocine+propofol （group C）， flurbiprofen axetil+propofol （group D）， respectively. Lab indicator， anesthesia and analgesia effect and ADR were compared among 4 groups after treatment. RESULTS： Perioperative DBP and postoperative HR of group D were higher than those group A， and perioperative SpO2 was higher than group A， B and C， with statistical significance （P＜0.05）； the propofol dosage of group D and B was significantly less than group A and C， with statistical significance （P＜0.05）. The recovery time of group D was markedly less than group A， with statistical significance （P＜0.05）. VAS score of group D was significantly lower than that of group A and C， with statistical significance （P＜0.01 or P＜0.05）. The satisfactory degree of groups was better than group A， with statistical significance （P＜0.05）； the number of ADR cases in group D was less than other groups， with statistical significance （P＜0.05）. CONCLUSIONS： Flurbiprofen axetil has significant analgesic effect， which could reduce the dose of propofol， shorten the recovery time， and reduce the incidence of ADR in painless induced abortion， it is a safe and effective anesthesia regimen. |
