我国社区2型糖尿病患者用药依从性影响因素研究

| 摘要： | 目的：为提高我国社区2型糖尿病患者的用药依从性和治疗效果提供参考。方法：利用北京大学中国卫生经济研究中心“中国糖尿病药物经济学研究”课题组的随访数据，采用单因素分析、Logistic回归模型等对用药依从性进行单因素分析和多因素分析。结果：共纳入2 236例我国社区2型糖尿病患者，用药依从性好的患者仅占23.48％。家庭平均月收入和病程两个因素对用药依从性有显著影响。其中，家庭平均月收入≥6 000元以上的患者和家庭平均月收入3 000～＜6 000元的患者相对于家庭平均月收入＜3 000元的患者，其用药依从性好的发生率分别为70.8％[95％置信区间（CI）（0.532，0.944）]和78.5％[95％CI（0.614，1.004）]，即当对于家庭平均月收入＜3 000元的患者，病程5～10年的患者和病程＞10年的患者相对于病程＜5年患者，其用药依从性好的发生率分别为69.0％[95％CI（0.542，0.878）]和59.1％[95％CI（0.459，0.760）]。结论：我国2型糖尿病患者的用药依从性总体上不理想，且家庭平均月收入较高和病程较长的患者用药依从性较差。 |
| ABSTRACT： | OBJECTIVE： To provide reference for improving medication compliance and therapeutic efficacy in community patients with type 2 diabetes mellitus （T2DM） in China. METHODS： The data was from the “Economic Study of Diabetic Medications in Patients with T2DM in China”， which was conducted by China Center for Health Economic Research of Peking University. Both univariate analysis and Logistic regression model were used for single factor analysis and multiple factor analysis of medication compliance. RESULTS： Totally 2 236 community T2DM patients were included. The rate of good medication compliance was estimated at as low as 23.48％. Monthly household income and duration of the disease had significant impact on medication compliance. Incidence of good the medication compliance for patients with monthly household income ≥6 000 yuan and during 3 000-＜6 000 yuan， compared to those with monthly household income ＜3 000 yuan， were 70.8％ [95％CI（0.532，0.944）] and 78.5％ [95％CI（0.614，1.004）] respectively. Incidence of good medication compliance for the patients with 5 to 10 years and ＞10 years of disease duration， compared to those with ＜5 years of disease duration， were 69.0％ [95％CI（0.542， 0.878）] and 59.1％ [95％CI（0.459， 0.760）] respectively. CONCLUSIONS： The medication compliance among community patients with T2DM in China was generally poor. Longer duration of the disease and higher monthly household income are associated with poorer medication adherence. |
