TITLE:
摘要: 目的:为临床预防抗肿瘤药物相关用药错误的发生提供参考。方法:检索PubMed、Embase、中国期刊全文数据库(CNKI)、万方数据库、中文科技期刊数据库(VIP),收集国内外抗肿瘤药物相关用药错误文献,总结和分析用药错误涉及的药物、原因、类型及转归情况。结果:共纳入有效文献65篇,包含78例患者。发生单种药物用药错误例数排名前4的药物为长春新碱(28例,35.9%)、甲氨蝶呤(24例,30.8%)、顺铂(6例,7.7%)、阿糖胞苷(5例,6.4%),而发生多种药物用药错误的有5例(6.4%)。51例患者(65.4%)为鞘内注射相关用药错误,27例患者(34.6%)为非鞘内注射相关用药错误;用药错误类型排名前3的是用药过量(38例,48.7%)、给药途径错误(29例,37.2%)、药品错误(7例,9.0%)。45例患者(57.7%)完全恢复正常,19例患者(24.4%)因用药错误最终导致死亡。结论:抗肿瘤药物相关用药错误不仅影响了患者的治疗进度,还可能导致严重的毒副反应甚至死亡,因此需要采取多种方法从根本上避免抗肿瘤药物相关用药错误的发生。
ABSTRACT: OBJECTIVE: To provide reference for preventing the occurrence of medication errors related to antitumor drugs in clinic. METHODS: Retrieved from PubMed, Embase, CNKI, Wanfang database and VIP, domestic and foreign literatures about medication errors related to antitumor drugs were collected. Related drugs, causes, type and outcome of medication errors were analyzed and summarized. RESULTS: Totally 65 valid literatures were included, involving 78 cases. Top 4 drugs in the list of case number of medication error were vincristine (28 cases, 35.9%), methotrexate (24 cases, 30.8%), cisplatin (6 cases, 7.7%), and cytarabine (5 cases, 4.4%). But there were 5 cases of medication errors related to various drugs. There were 51 cases of intrathecal injection (65.4%) and 27 cases of non-intrathecal injection(34.6%). Top 3 types of medication errors were over dosage (38 cases, 48.7%), wrong route of administration (29 cases, 37.2%), drug errors (7 cases, 9.0%). Forty-five cases returned to normal completely (57.7%). There were still 19 patients (24.4%) eventually turning to death because of medication errors. CONCLUSIONS: medication errors related to antitumor drugs in clinic not only affect the patient’s therapy progress, but also lead to serious toxic reactions and even death. Measures should be taken to avoid the occurrence of medication errors associated with antineoplastic drugs.
