| 摘要： | 目的：评价我院的质子泵抑制剂在合理用药专项整治前后的使用情况，为其在临床上经济、合理、有效地使用提供参考，同时探索有效的合理用药管控手段。 方法：随机抽取太原市中心医院2015年（整治前）的1 172份及2017年上半年（整治后）的588份使用质子泵抑制剂的患者病历，采集患者的相关用药信息，从适应证、药物品种遴选、药物用法用量、用药疗程、是否重复用药等方面对质子泵抑制剂的使用合理性进行评价和分析。结果与结论：针对出现的问题，我院采取的主要管控手段包括开展合理用药宣讲以提升医务人员对合理用药的全面认知；通过专项研讨与培训加强医护人员对药物的基本认识；定期进行各科室用药互评以加强医护人员临床用药能力；通过成立专项用药点评小组每月进行点评并即时公示结果、重点科室重点监控，以此促进药物合理应用；为医护人员和患者提供用药咨询和床边药学服务，及时解决用药过程中遇到的问题；用药点评结果与科室、医师绩效挂钩，通过惩处方式加强合理用药监管等。2015年的质子泵抑制剂使用不合理率为31.9％，经2016年合理用药专项整治后，2017年上半年的质子泵抑制剂使用不合理率降为12.1％。 |
| ABSTRACT： | OBJECTIVE： To evaluate the use of proton pump inhibitors （PPIs） in our hospital before and after the special rectification of rational drug use， provide reference for economical， reasonable and effective use of PPIs and explore the effective means of rational drug use control. METHODS： Medical records of 1 172 patients receiving PPIs were randomly selected from Taiyuan Central Hospital in 2015 （before rectification）， and 588 medical records were collected in the first half of 2017 （after rectification）. The medication information of patients were collected. The rationality of PPIs use was evaluated and analyzed in respects of indications， selection of drug varieties， usage and dosage， medication course， repeated medication， etc. RESULTS & CONCLUSIONS： For the problems that occurred， our hospital took the main management and control means including that rational drug use education was carried out to promote comprehensive cognition of medical staff to rational drug use； special discussion and training strengthen basic understanding of medical staff to drugs； The ability of medical staff using drugs was strengthened by regular mutual evaluation of drug use among various departments； Rational drug use was promoted by setting up special drug evaluation team for monthly evaluation and immediate publicity and key monitoring for key department； the medication consultation and bedside pharmaceutical care was provided for medical staff and patients so as to solve problems encountered in the course of medication； the results of drug evaluation are linked to performance， and supervision is strengthened through punishment.The unreasonable rate of using PPIs in 2015 was 31.9％. After special rectification in 2016， the unreasonable rate of using PPIs dropped to 12.1％ in the first half of 2017. |
