TITLE：
摘要：	目的：分析和总结华法林与抗肿瘤药物和相关辅助药物的相互作用，为临床安全用药提供参考。方法：采用回顾性分析的方法，通过医院信息管理系统调取2006年1月－2015年12月我院住院患者使用华法林的医嘱，提取患者基本信息、适应证、华法林用法用量、国际标准化比值（INR）等，并依据MICROMEDEX 2.0数据库中的药物相互作用评价标准予以总结，并查阅相关文献予以确证。结果：共纳入72条医嘱，合计56例患者，其中14条医嘱在抗肿瘤治疗过程中华法林的用药剂量有调整，推断可能发生了相互作用。MICROMEDEX 2.0评价共有17种药物与华法林存在相互作用，等级为“严重”的有厄洛替尼、氟尿嘧啶、卡培他滨、依托泊苷、卡铂、长春新碱、布洛芬。结论：华法林与多种抗肿瘤药物和相关辅助药物存在相互作用。临床在华法林与肿瘤化疗药物和相关辅助药物联用时，应严密监测患者INR值，做好预防措施，保障患者的用药安全。
ABSTRACT：	OBJECTIVE： To analyze and summarize the drug interaction of warfarin with antitumor drugs and adjunctive drugs， and to provide reference for safety of clinical drug use. METHODS： By retrospective analysis， medical orders of inpatient receiving warfarin were collected from hospital information system during Jan. 2006－Dec. 2015. General information of patients，indications，usage and dosage of warfarin，INR values were extracted and summarizcd according to drug interacien evaluation criteria of MICROMEDEX 2.0 database， and then confirmed by reviewing related literatures. RESULTS： Totally 72 medical orders were included，involving 56 patients. Dosage of warfarin in 14 medical orders had adjusted in antitumor treatment. MICROMEDEX 2.0 evaluation involved drug interaction of 17 drugs with warfarin. Among them， erlotinib， fluorouracil， capecitabine， etoposide， carboplatin， vincristine and ibuprofen would seriously interact with warfarin. CONCLUSIONS： Warfarin interacts with a variety of antitumor drugs and adjunctive drugs. When warfarin combine with antitumor drugs and adjunctive drugs， medical workers should closely monitor INR values， and complete the prevention measures to ensure the safety of drug use.
