TITLE: Research progress on traditional Chinese medicine for the prevention and treatment of asthenozoospermia based on mitochondrial autophagy
摘要: 弱精子症是男性不育的重要诱因之一,病因复杂,目前尚无特异性靶向治疗药物。线粒体自噬在该疾病进展中发挥关键作用。本文系统梳理了线粒体自噬分类、线粒体自噬介导弱精子症的相关机制,总结了中药单体与中药复方干预弱精子症的研究进展,结果显示,线粒体自噬作为细胞质量控制体系的重要组成部分,主要包括泛素依赖性与非泛素依赖性两种类型,其中以PTEN诱导假定激酶1/Parkin信号通路介导的泛素依赖性线粒体自噬研究最为经典,该通路可通过降解受损线粒体、维持线粒体功能稳态,参与调控精子生理功能,拮抗精子发生相关蛋白33异常所致的精子线粒体损伤,在弱精子症的疾病进展中发挥关键的双向调控作用。中药单体(如大蒜素、丁香酚等)及复方(如补肾强精颗粒、黄精赞育胶囊等)可根据不同病理状态靶向激活或抑制线粒体自噬、修复受损线粒体功能、改善精子质量及活力,充分体现出中药双向调节“、以平为期”的独特治疗特色。
ABSTRACT: Asthenozoospermia is one of the important causes of male infertility, and its etiology is complex. Currently, there are no specific targeted therapeutic drugs available. Mitochondrial autophagy plays a crucial role in the progression of this disease. This article systematically summarizes the classification of mitochondrial autophagy and the mechanisms of mitochondrial autophagy mediated asthenozoospermia, and summarizes the research progress of traditional Chinese medicine monomers and formulas in intervening in asthenozoospermia. The results show that mitochondrial autophagy, as an important component of cellular quality control system, mainly includes two types: ubiquitin-dependent and non ubiquitin dependent. Among them, PTEN induced kinase 1/ Parkin signaling pathway mediated ubiquitin dependent mitochondrial autophagy is the most classic. This pathway can participate in regulating sperm physiological functions by degrading damaged mitochondria, maintaining mitochondrial functional homeostasis, antagonizing sperm mitochondrial damage caused by spermatogenesis-associated protein 33 abnormalities, and playing a key bidirectional regulatory role in the progression of asthenozoospermia. Traditional Chinese medicine monomers (allicin, eugenol, etc.) and compound formulas (Bushen qiangjing granules, Huangjing zanyu capsules, etc.) can target and activate or inhibit mitochondrial autophagy according to different pathological states, repair damaged mitochondrial function, improve sperm quality and vitality, fully reflecting the unique therapeutic characteristics of traditional Chinese medicine’s dual directional regulation and “balance‑oriented approach”.
