目的：降低医院药房单人调剂风险，提高急诊药房药学服务质量。方法：分析我院急诊药房单人调剂处方风险的环境因

素、人为因素、社会因素，提出改进建议和措施，建立风险干预机制，并评价管理效果。结果：通过改进药房软硬件、落实相关制度、

加强培训学习、改进调剂流程、改善窗口秩序、加强药品管理、实施踪近错误管理等措施后，与管理前（2014 年10－12 月）比较，管

理后（2015 年2－4 月）踪近错误、纠纷次数显著降低（P＜0.05），给药错误降低为0。结论：单人调剂处方时通过加强风险管理、提

高药师处方审核能力，可提高急诊药房调剂药学服务。

OBJECTIVE：To reduce the risk of single dispensing，and to improve the quality of pharmaceutical care in emergency

pharmacy. METHODS：Single dispensing environmental factors，human factors and social factors of emergency pharmacy in our

hospital were analyzed；the suggestions and measures were put forward，risk intervention mechanism was established and management

effects were evaluated. RESULTS：Through improving the pharmacy hardware and software，carrying out the relevant system，

enhancing training，improving the process of dispensing and window order，promoting the drug management and carrying out

near error management. Compared with before management（Oct.-Dec. 2014），the times of near error and disputes significantly reduced

after management（Feb.-Apr. 2015）（P＜0.05），and medication error reduced to zero. CONCLUSIONS：As single dispensing，

risk management and the improvement of prescription audit ability can improve pharmaceutical care in emergency pharmacy.
