TITLE：
摘要：	目的：提升我院高危药品安全管理水平。方法：依据国际医疗卫生机构认证联合委员会（JCI）的认证标准，针对我院临床高危药品管理中存在的问题，采用PDCA（计划、执行、检查、处理）循环方法及反向鱼骨图法等制定相关对策并实施，评价实施后12个月内29个科室的遵从率。结果：针对反向鱼骨图法得到的2个负面因素制定了相应的应变措施；通过成立高危药品管理相关组织、制定高危药品管理制度和质量衡量标准、加强对相关人员的培训与宣教、完善医院信息系统等措施，对高危药品实施全程、有序的质量控制。各科室对标准的遵从率从1月的53％上升至6月时的100％（P＜0.05），并持续稳定至12月。结论：采用PDCA循环方法加强了我院临床高危药品管理的制度化、规范化及多部门协作能力，促进了患者用药安全。
ABSTRACT：	OBJECTIVE： To improve high-alert medication management in our hospital. METHODS： According to assessment criteria of JCI， referring to the problems of high-alert medication management in our hospital， PDCA （plan， do， check， action） cycle method， reverse fishbone diagram and other methods were adopted to formulate and implement relevant countermeasures. Compliance rate of 29 departments was evaluated within 12 months after the implementation of countermeasures. RESULTS： Relevant emergency measures were formulated for 2 negative factors by reverse fishbone diagram； whole-course and orderly quality control of high-alert medication was conducted through establishing high-alert medication management institutions， formulating high-alert medication management system and quality measurement criteria， strengthening staff training and education， improving hospital information system， etc. The compliance rate of departments increased from 53％ in Jan. to 100％ in Jun. （P＜0.05）， and kept stable until Dec. CONCLUSIONS： PDCA cycle improves the systemization and standardization of high-alert medication management and multiple department cooperation， and contributes to the safety of drug use.
