目的:优化我院静脉药物集中调配中心(PIVAS)工作流程的各操作环节,提高配制质量。方法:将精益管理理论应用于

PIVAS各环节,找出影响配制质量的问题并提出改善措施及持续改进;对管理前后各10 个月的相关指标进行比较以评价改进效

果。结果与结论:PIVAS通过实施包括优化审方软件、合理排班、人员培训、目视管理、“5S”管理、设备创新和精选耗材等措施,将

每日平均工作时间由8 h 缩短至6.5 h,工作人数由52 人减少至48 人,差错率由1.76‰降低至0.84‰,药品库存平均周转天数由

7.42 d 降低至4.93 d,医嘱不合格率由1.28‰降低至0.59‰。通过实行精益管理,加强了医嘱用药合理性,减少了各种差错,提高了

工作效率,提升了配制质量,保障了临床用药的及时、安全、有效。

ABSTRACT:

OBJECTIVE:To optimize each link of work flow in PIVAS of our hospital,and to improve the quality of drug dispensing.

METHODS:Lean management was applied in each link of PIVAS,the problems that influenced the quality of drug dispensing

were found out,and then countermeasures were put forward for continuous improvement. Related indicators were compared

10 months before and after management in order to evaluate improvement effects. RESULTS & CONCLUSIONS:Through

implementing various measures in PIVAS,such as optimizing prescription checking software,rational staff arrangement,staff training,

visual management,“5S”management,equipment innovation and careful-chosen consumptive material,the average daily

working time reduced from 8 h to 6.5 h,the staff number from 52 to 48,the error rate from 1.76‰ to 0.84‰,the average drug inventory

turnover days from 7.42 days to 4.93 days,and the prescription failure rate from 1.28‰ to 0.59‰. Lean management reduces

various kinds of mistake,improves work efficiency,increases dispensing quality,strengthens rationality of drug use and ensures

the effectiveness and safety of clinical drug use.
